Do you have: United Healthcare (UHC)*, First Health, Imagine Health, Sutter Health, VA CCN- Optum East, or Velosity PPO; or do you have the Multiplan or PNOA logos on your insurance card displayed below?
Our collaboration with The Lactation Network (TLN) allows us to provide lactation visits covered 100% (with no out of pocket costs**) to many individuals insured with these plans. To check your coverage with TLN, click the button below to fill out their short form. The Lactation Network will email you to inform you of your coverage promptly, often within one business day.
Self pay rates:
$195 Initial in home visit ***
$155 Follow up visit ***
$125 Virtual visit
* Tera’s Lactation Care, LLC is in-network through TLN for Commercial members with UnitedHealthcare in 49 states (Hawaii pending) for in-person consultations.
** Number of covered visits varies based on insurance provider
*** Please note, an additional $50-100 travel fee will be added for drive times greater than 30 minutes from Mequon, WI to be determinted by Tera’s Lactation Care, LLC
Following our visit, and once payment is received, I will provide you with a super bill for you to submit to your insurance company to seek reimbursement. Please see our FAQ page for more information on your rights to covered lactation services.
